Personalized Medical Emergency Identification Cards are made to order. Made of durable plastic (PVC) material with full color print on the front side and black color on the back side. Will not fade or crack.

Name section ( up to 4 lines), Emergency contacts ( up to 3 lines), medical contacts ( up to 2 lines). Information on the back will be placed in one line separated by dashes or one per line if space allows

In case of emergency first responders or ER personnel will have full history of your conditions, medications, allergies and emergency contact information.
Once you purchase, please send us a message with the following information:

We welcome all returns within 30 days. Please contact us prior to shipping your item back to us to obtain RMA number. Item must be in original package, unused and in original condition to receive full refund. Refunds will be issued in the same form as payment made to us. Items marked as FREE SHIPPING will receive refund minus actual shipping cost. Buyer is responsible for return shipping.
